How to fill out a W-9 form

The W-9 form requires details such as your name, address, and TIN, which can be either your Social Security number (SSN) or Employer Identification number (EIN), depending on whether you’re operating as an individual or a business.

  1. Name: Enter your full legal name. If you are an individual, use your personal name. If you are a business entity, use the registered business name.
  2. Business name/disregarded entity name: If applicable, enter your business name or disregarded entity name if it is different from the name you provided in the first section.
  3. Federal tax classification: Check the box that corresponds to your federal tax classification. This could be individual/sole proprietor, C corporation, S corporation, partnership, trust/estate, or limited liability company (LLC).
  4. Exemptions: If you are exempt from backup withholding or Foreign Account Tax Compliance Act (FATCA) reporting, provide that information here. Most individuals and small businesses will leave this section blank.
  5. Address: Enter your complete mailing address, including street, city, state, and ZIP code. This is the address where you will receive any tax documents.
  6. Taxpayer Identification Number (TIN): Provide your Social Security number (SSN) if you are an individual or sole proprietor. If you are a business entity, provide your Employer Identification number (EIN).
  7. Certification: Read the certification statement carefully. By signing the form, you certify that the information provided is correct and that you are not subject to backup withholding. Sign and date the form.
  8. Optional section: You may include additional contact information, such as a phone number or email address, in the space provided.
